Know Your Plan: Know your medicines!
Here are 4 key steps you need to know to create your personal medication record.
Step 1: Make a list of all of the medicines (and dosages) you are currently taking. Include medicines that you may take from time to time (such as allergy medications or prescription pain relievers).
Step 2: Note whether you take the generic or brand name of each medicine.
Step 3: Write down how often you take each medicine, who prescribed it to you and when, and for what reason.
Step 4: Be sure to include any over-the-counter medicines, vitamins, minerals, and other dietary supplements you regularly take.
This is called your personal medication record, and it will make it much easier to work with your new Medicare prescription drug plan. It will also help you partner with your health care providers to talk about new treatment options, food and drug interactions, and your general health.
