Know Your Plan: Know your medicines!
A personal medication record make navigating your pharmacy benefit much easier, whether you're choosing a new plan or working with one that you've used for years. It will also help you partner with your doctor to talk about new treatment options, food and drug interactions, and your general health.
Step 1: Make a list of all of the medicines (and dosages) you and members of your family are currently taking. Include medicines that you may take from time to time (such as allergy medications or pain relievers).
Step 2: Note whether you take the generic or brand name of the medicine.
Step 3: Write down how often you take each medicine, who prescribed it to you and when, and for what reason.
Step 4: Be sure to include any over-the-counter medicines, vitamins, minerals, and other dietary supplements you regularly take.
